Arranging your Care and Support

You will have a face-to-face meeting with a management team member to go through your needs, wants and goals. After that, we collaborate with you and your support system to create a care package tailored to your needs. An evaluation of the risks will be part of the initial meeting to guarantee the continued safety of you and our employees.

After this meeting, we’ll work with you to choose a small staff team to look after you. Weekly contact will be maintained between you and our management team to determine whether you are satisfied with the assistance you are receiving.

After the first four weeks, we will evaluate to see if any modifications are necessary. If you have a social worker, we will work with them to let them know about the changes that need to be made to your care package.

Dynamic Social Care Ltd works jointly with community health and social services departments to ensure that the needs of the adults we support are met. We do so by ensuring that people are referred to both organizations when there is a change to their needs or in situations where there is crisis and concerns of people’s safety relating to safeguarding and deprivation of liberty.

Paying for your care and support Local Authority Funding and Direct Payments

If you have been assessed as needing help from social services, a financial assessment will be completed by the Local Authority to determine how much you contribute towards your care. You can choose to have your personal budget managed by the Local Authority or to receive direct payments to help you pay for your own care. This gives you more choice, flexibility and control about which care provider to choose.

NHS Continuing Healthcare

NHS Continuing Healthcare is the name given to a package of care arranged and funded by the NHS for people with complex ongoing health needs. If you are eligible, CHC is completely free. To be eligible, you must be over 18 years of age and have “a complex medical condition and substantial and ongoing care needs”. You must have a “primary health need” which means that your main need for care must relate to your health.

There is an initial screening assessment called the Checklist Tool. If this screening suggests that you may be eligible for CHC funding, you will be referred for a full up to date assessment of your needs. If you are not assessed as eligible, other options of care are provided by the local authority. If you do have health needs, the NHS may still pay for part of your care, sometimes known as a joint package of care. You will need to contact your local Clinical Commissioning Group.

Private Funding

If you do not qualify for council funding, or you choose not to be financially supported by the Local Authority, you can arrange to pay for your own care direct with us. Please get in touch for our costings.
